New research shows the Ketogenic diet has superior potential for improving metabolic health, reducing liver fat, and managing prediabetes.

A comprehensive study conducted by researchers at the Washington University School of Medicine in St. Louis and the University of California, Berkeley, published in the journal Cell Metabolism, has revealed significant breakthroughs in improving metabolic health through nutritional intervention.

The randomized clinical trial focused on adults with obesity, prediabetes, and liver fat accumulation. Participants were divided into three groups following distinct dietary patterns: Ketogenic, Mediterranean, and low-fat, plant-based diets. To ensure scientific rigor, researchers provided all meals and closely monitored adherence over a four-month period, targeting a 10% reduction in initial body weight.

Although all three groups achieved similar weight loss, the internal metabolic transformations differed significantly. Magnetic Resonance Imaging (MRI) results indicated that the Ketogenic group achieved an average reduction of 67% in liver fat, compared to 45% in the other groups. Furthermore, hepatic insulin sensitivity in the Ketogenic group improved two to three-fold, significantly inhibiting de novo lipogenesis in the liver.

Regarding glycemic indices, after the trial, approximately 50% of participants in the Ketogenic group no longer met the clinical criteria for prediabetes. This rate was 29% for the Mediterranean group and only 7% for the low-fat group. The 24-hour average blood glucose levels in the Ketogenic group dropped by 20%, outperforming the 8% reduction observed in the comparison groups.

Professor Samuel Klein, the study lead, emphasized that minimizing carbohydrate intake provides supplemental benefits for glucose and lipid metabolism beyond mere weight loss. Notably, despite high fat consumption, participants' cholesterol and blood lipid levels did not increase, a finding that surprised the research team.

However, registered dietitian Ilana Muhlstein advises that the Ketogenic diet should be viewed as a short-term intervention. Due to high saturated fat content and low fiber intake, it is not recommended for long-term cardiovascular health. Instead, patients might consider it as an initial approach for rapid results, subsequently transitioning to the Mediterranean diet for sustainable, long-term health.
